Support with social, financial, mental health and practical challenges.

While your GP and practice nurse offer treatments for medical issues, they often don’t have time to explore other types of support with their patients. This is where our Community Link Workers come in.

We’re able to spend time exploring the resources and techniques that may be helpful to you. We can help with challenges such as:

  • Bereavement
  • Loneliness
  • Relationship problems
  • Feeling low or stressed
  • Money
  • Employment
  • Housing
  • Caring for a family member
  • Parenthood
  • Experiencing violence or abuse
  • Living with a long-term condition (like chronic pain, COPD, or diabetes)
  • Having a disability

Who are Community Link Workers?

Our Community Link Workers are qualified health and social care professionals, who all have experience supporting people in the community. 

They are excellent listeners and they will take time to understand your needs and connect you with resources in our local community.

Your Community Link Worker will have access to your medical records and adhere to strict confidentiality guidelines. If you would prefer for your Community Link Worker to not access your records, you can speak to a member of staff at your GP surgery.

What to expect

You'll find a warm welcome and free non-judgemental support and advice from our Community Links workers.

Our Community Link Workers are friendly, non-judgemental people who will listen to you and offer you support, whatever challenges you are facing.

You can expect to have up to four meetings with us. These can be face to face, over the phone or via a video link – whatever works for you.

Community Link Workers will always be guided by your goals and priorities. You’ll be able to take things at your own pace and you’ll never have to do anything you don’t feel comfortable with.

We won't share your information with anyone else unless:

  • you give us permission

  • we're concerned about your safety or the safety of someone else

You'll find us at every local GP surgery in Renfrewshire. 

To make an appointment:

Within one week of receiving your referral, we will speak to you by phone to determine the best support for you.

During this phone call, we may be able to provide you with information about support that is available from other organisations in Renfrewshire.

If you need more in-depth support, we will schedule an appointment with a Community Link Worker. You will be able to choose whether this is face-to-face, by phone, or online. 

As well as holding one-to-one sessions with patients, we deliver three wellbeing groups. Get in touch with us to find out more.

  • Mixed anxiety and depression: Participants will learn strategies for tackling symptoms of anxiety and depression. 
  • Living Life to the Full: Participants will develop simple and practical techniques to overcome stress, anxiety and low mood.
  • Reclaim your Life: Designed for those living with long-term conditions, this programme helps participants find new ways of managing their illness, pain or fatigue.
